All students will receive vision, hearing and scoliosis screening according to NYS guidelines.

Physicals/Health Examination

NYS Health Examination Overview
The Physical Exam Form is required for all students who are entering SSPJ for the first time as well as for students entering Kindergarten, Grades 1, 3, 5, and 7.

Immunizations
-
A record of immunizations is due at the time of application/registration. This record will be reviewed by the school nurse and checked for compliance to New York State guidelines. Please present a new record when additional vaccines are administered at the doctor's office.

Dental
-
A Dental Certificate is requested at the same time for Kindergarten, Grades 1,3,5,7, and all new students.

Middle School Sports (CMSAA)
-
Documentation of physical exam, dated within the calendar year, must be on file signed by a licensed practitioner. Additionally, a Health Screening/Medical Update form must be completed by the parent.

Medications

If your child requires regular, over-the-counter or emergency medication in school, please contact the school nurse to have the required forms completed.
